About the Designated User Form

UNL faculty, corporate users or persons with disabilities can designate up to four people as proxies to check out or pick up library materials using their NCard or community user library card.

Designated users must be at least 18 years of age.
Designated users must have a current NCard or Community User library card.
The authorizing person and designated user must have no fines on their library record.

Complete the form below to assign new designated users or renew current designated users. Allow at least 24 hours for processing. A paper version of this form is available at all ASKus locations.
